COVID-19 Protocol for DCL Site Visitors
As the coronavirus (COVID-19) pandemic continues, we are implementing a strict visiting protocol for all of our facilities. The safety of our employees, visitors, and constituents is our overriding priority. We are following guidance from the Centers for Disease Control and Prevention and local health authorities to prevent the spread of the coronavirus and reduce the potential risk of exposure to our workforce.
To visit a DCL facility you must review the protocol below, fill out our visitor form within 24 hours of visit, and provide proof of a negative COVID-19 test taken within 72 hours of your visit.
Review and abide by the health and safety protocols outlined below.
Fill out the visitor form within 24 hours of your visit, which includes acknowledgement of a negative test result.
Visitors will be allowed to enter the facility only if they are “asymptomatic” for the duration of their visit. All visitors must comply with all prevention protocols, which include the following:
Alert your DCL contact immediately if you have any sickness symptoms.
Take a temperature scan upon entering the facility. This is required every time you enter, even if it's multiple times per day or week.
Wear a facial covering (mask) over their nose and mouth, regardless of vaccination status, at all times while on the facility premise, except while eating or drinking.
Maintain a minimum of six feet of distance from other workers in the workplace. Do not shake hands or make direct contact with any other employees/visitors.
Wash your hands frequently. Hand sanitizer is also available throughout the facility.
Anyone entering our facilities will encounter temperature checkpoints and complete a temperature scan. Temperature scan results must be below 100.4 F. Any visitor with temperature results greater than 100.4 F will be restricted from entering the facility.
Complete the COVID Screening Questions
Visitors will be required to complete the following COVID-19 Visitor Screening Questionnaire upon going through security each day. If any questions are answered with a “YES” response, then access to DCL facilities will not be granted.
Please respond to each of the following questions truthfully and to the best of your ability:
Have you developed/experienced any of the following symptoms?
fever or chills • cough • shortness of breath or difficulty breathing • fatigue • muscle or body aches • headache • new loss of taste or smell • sore throat • congestion or runny nose • nausea or vomiting • diarrhea
Are you isolating or quarantining because you tested positive for COVID-19 or are worried that you may be sick with COVID-19?
Have you been in close physical contact with a confirmed COVID-19 case or symptomatic person(s)
Anyone who is known to have laboratory-confirmed COVID-19?
Anyone who has any symptoms consistent with COVID-19?
Are you or a member(s) of your household currently waiting on the results of a COVID-19 test?
Have you or someone you’ve been in contact with attended a gathering where proper social distancing protocol was not followed?
DCL understands that you may have traveled to make your scheduled visit and were required to take a laboratory confirmed diagnostic or antigen test. Were the results “Positive?”
COVID-19 Testing Protocol
Visitors will be required to acknowledge one of the following COVID-19 test results:
A “NEGATIVE” laboratory confirmed diagnostic test. The test required to be taken within three days of the on-site visit.
A ”NEGATIVE” antigen test result the day of the on-site visit.
